How to avoid cramming for exams? Help me make a revision schedule please?

I'm in my final year at university and have two science exams and a maths exam coming up in about a month. I always end up steadily revising but never getting it finished in time so I end up cramming last minute, which I don't want to do this time. I've always got good marks on exams but I want to get exceptional marks this year so that I end up with a first cl degree. What advice would you give please for how to make a revision schedule? Obviously I will be revising daily and all day every day for the next month, with short breaks. I need to work on science a lot more than I do maths so I'll obviously spend more time on that. What advice would you offer please? Also, if you have any effective revision tips you want to share, go ahead:) I seem to learn well visually so advice for visual learners would be helpful. I also record myself sometimes and repeat things over and over which can help but it's time consuming. Anyone know any quicker methods to retain info and how to set up a revision schedule so I get things done in time? I've been going over work all year so I'm not starting from scratch, but there's still a lot to cover. Help please asap! Thanks:)
