Im applying for a bank certification at BDO Bank, philippines..I have a question?

Im applying for a bank certification here at BDO, needed so that I can apply to be a lotto agent. This might sound stupid, but what do I put for "Benificiary/payee" on the bank certification applicaton form? Do i put the PCSO lotto as the benificiary, my name? What? Thank you
